| Every Wednesday, Adult and Children, $12 fee for children's vaccine provided by the state. Adult vaccine and private pay children's vaccine - call for prices. Begins 8:30 a.m. by appointment only (989) 743-2355. If a parent is not accompanying child to immunization appointment, please call in advance to receive consent form.
