  • PUBLIC HEALTH IS THAT AREA OF HEALTH CARE THAT CONCERNS ITSELF WITH THE HEALTH STATUS OF THE ENTIRE POPULATION.

Legal Mandate
  • Michigan's constitution of 1963 states "The public health and general welfare of the people of the state are hereby declared to be matters of primary public concern.  The legislature shall pass suitable laws for the protection and promotion of public health."

Legal Mandate
  • Michigan's Public Health Code, Act 368 of 1978, as amended, describes the responsibilities of state and local health departments as follows "Continually and diligently endeavor to prevent disease, prolong life, and promote the public health through organized programs, including prevention and control of environmental health hazards; prevention and control of diseases; prevention and control of health problems of particularly vulnerable population groups; development of health care facilities and agencies and health services delivery systems; and regulation of health care facilities and agencies and health service delivery systems to the extent provided by law." (Sections 2221 & 2433 of Act 368, 1978, as amended.)

COST RECOVERY

  •    IN ORDER TO RECOVER THE $2.9 MILLION IN ANNUAL FUNDING, OPERATING COSTS ARE ALLOCATED TO 54 DIFFERENT COST CENTERS AND THEN REPORTED AND  BILLED TO 17 DISTINCT FUNDING SOURCES.

 Final Comments on Finances:
  • While the $292,172 General Fund contribution represents a small portion of the County General Fund Budget (1.5%), it is an indispensable foundation upon which $2.6 million in additional Public Health funding is generated to protect the health of Shiawassee County residents.


  • From an economic standpoint, the County's General Fund contribution yields over 867% return on investment for Health Care Service delivery in Shiawassee County.  Secondly, the bulk of the Department’s $2.9 million budget is spent in Shiawassee County, thereby bolstering the local economy. In addition, 75% of the Health Department’s budget is generated from sources outside of the County.  Finally, the Health Department provides thirty-five good paying jobs for professional technical and clerical staff in our community at a time when the State’s economy continues to struggle.

Challenges and Opportunities:
  • Federal, state and county budget constraints will require us to continue to critically rethink how we do business.
  • There is a limited supply of nurses and sanitarians; these knowledge workers  form the backbone of our professional staff and our capacity to effectively serve the public.
  • The demand for and the cost of health care are both rapidly increasing.

Challenges and Opportunities:
  • Emergency Preparedness issues such as Pandemic Influenza and Bioterrorism (Smallpox and Anthrax attacks) focus public  attention on the importance of public health while diverting scarce resources from our existing services and needs.

  • Bad habits of over-eating the wrong things and lack of physical activity have become part of American culture; Public Health needs to find ways to turn back the tide of 40 to 50 years of gradually increasing poor health behaviors by imagination, innovation and thinking outside the box to reverse this sad paradigm.

Challenges and Opportunities:
  • Reductions in sickness and death due to infectious diseases and the post war baby boom has resulted in an older population that requires public health to focus on chronic diseases such as heart disease, cancer, etc. These chronic diseases are the major killers in Shiawassee County.

Challenges and Opportunities:
  • Population growth coupled with past and present human activities has resulted in environmental deterioration; especially in the areas of drinking water and managing sewage control.
  • Some infectious diseases, such as tuberculosis, are beginning to reappear in drug resistant forms.
  • The high cost of health care insurance and the ability of employers to continue paying for it in a slow economy pose additional problems for accessing health care.

Challenges and Opportunities:
  • The burden of mental illness on physical health has been profoundly underestimated. Mental health is a public health issue because it is indispensable to personal well being, family and interpersonal well being and relationships.
  • As health care becomes more complex, so does the potential for medical errors. This problem needs to be treated from a systems standpoint not as an individual issue.

  • Technology, information and communications are a double edged sword because advancements in these areas can be used in both beneficial and harmful ways. Potential confidentially breaches and security efforts to prevent them increase health care costs while these new technologies present opportunities for cost reductions, improved efficiencies and new hope for many victims of disease.

  • Managed care is another two edged sword replacing compassion with cost containment, services with products, and stakeholders with stockholders. The challenge is to realize the potential that these large organizations have to better prevent disease and promote health.

  • Lastly, public health needs to make use of the significant potential of genetics to improve health and prevent disease while avoiding discrimination, stigmatization and other societal harms which could result from its misuse.
